"Is the NPTE hard?" usually gets one of two answers: "you'll be fine" or a horror story. Neither helps you plan. FSBPT publishes enough data to give a better answer, and it depends a lot on who you are and how you prepare.

Is the NPTE hard?

The NPTE is demanding, but most US-educated candidates pass it on the first try: 85% in 2025, according to FSBPT. It's much harder for candidates educated outside the US, 39% of whom passed on the first try. Counting retakes, 98% of 2024 graduates of accredited US programs have passed.

In 2025, roughly one in seven US-educated first-time candidates didn't pass. That isn't a rare outcome, and it's worth preparing to avoid. But it's far from the coin flip some forum threads suggest.

Is the NPTE harder than it used to be?

US-educated first-time pass rates were 89% in 2021 and have been 84–86% since. FSBPT introduced a new content outline in January 2024, and the rate has held steady through it. The passing standard itself is criterion-referenced: you're measured against a fixed standard of entry-level competence, not graded on a curve against other candidates.

Your program matters, too

Program results vary widely. Across the 299 physical therapist programs in FSBPT's latest school report, the median two-year first-time pass rate is 87.4%, but 36 programs are below 70% and 118 are at 90% or above. What makes the NPTE hard

  • Breadth. The content outline runs from spinal cord injury levels to billing rules, statistics and wound dressings. Most programs don't cover everything to the same depth.
  • It tests reasoning, not recall. FSBPT's outline says the focus is "the clinical application of knowledge." The biggest question category asks you to evaluate and differentiate conditions, often in multi-question clinical scenarios.
  • "Best answer" questions. Several options are often reasonable. You're choosing the MOST appropriate or FIRST action, which punishes loose reading.
  • Stamina. Five hours and 225 questions, at about 80 seconds each, with only one break that doesn't cost exam time.
  • Unscored questions you can't identify. Forty-five pretest questions are mixed in, so some of the strangest questions you see may not even count. You can't know which ones.

Who finds the NPTE hardest

  • Candidates educated outside the US, whose first-time pass rate has been 39–45% over five years. FSBPT's own remediation advice for this group focuses on closing gaps against a CAPTE-equivalent education and on English-language skills.
  • Repeat takers. FSBPT says candidates who have failed twice or more are "unlikely to pass on any subsequent attempt" unless they undertake substantial remediation. See what happens if you fail the NPTE.
  • Anyone who studies passively. Rereading notes feels productive, but in a major review of learning research it rated low, while practice testing and spaced practice rated highest.

How to make the NPTE easier on yourself

Is the NPTE curved?

No. FSBPT uses a criterion-referenced passing standard of 600 on a 200–800 scale. Your result depends on meeting the standard, not on how other candidates did.

Is the NPTE harder than the NCLEX?

They're different exams for different professions, with different formats. The NPTE-PT is a fixed 225-question, five-hour exam; FSBPT and the NCLEX publisher report pass rates differently, so a direct comparison isn't meaningful.

What percentage of people fail the NPTE?

In 2025, 18% of all first-time NPTE-PT candidates didn't pass: 15% of US-educated and 61% of internationally educated candidates.

Is the NPTE harder than PT school exams?

It's broader. Program exams test one course at a time; the NPTE tests every system and non-system area at once, in five hours, with best-answer clinical reasoning questions.
