The National Physical Therapy Examination (NPTE) is the licensing exam every physical therapist in the United States has to pass. (Physical therapist assistants take a separate exam; see the NPTE-PTA guide.) It's a 225-question, five-hour, computer-based test run by the Federation of State Boards of Physical Therapy (FSBPT). This guide covers the whole exam, from what's on it to what happens after you click "end exam," and links to a deeper page on each part.

What is the NPTE?

The NPTE is the national licensing exam for physical therapists and physical therapist assistants in the US, developed and administered by FSBPT. The PT version (NPTE-PT) has 225 multiple-choice questions and takes five hours. Every US jurisdiction requires a passing score of 600 on a 200–800 scale, and it's one part of the licensure process alongside your state's own requirements.

FSBPT's NPTE Candidate Handbook describes two purposes for the exam: to make sure only people with the required physical therapy knowledge are licensed, and to give licensing boards a standard that's comparable from one jurisdiction to the next. The questions are written by volunteer licensed PTs and PTAs from a wide range of practice settings, and each one is reviewed by FSBPT's Exam Development Committee before it counts.

NPTE-PT format: how many questions and how long?

NPTE-PT exam format
Questions225 multiple choice: 180 scored + 45 unscored pretest
Sections5 sections of 45 questions
Testing time5 hours as one block (sections aren't timed separately)
Total appointment5 hours 30 minutes, including the scheduled break, tutorial and survey
BreaksScheduled 15-minute break after section 2 (not deducted from exam time); optional breaks after the other sections are deducted
Question typesStand-alone items (some with a graphic or video), plus up to 40 scenario-based questions at the end of sections
WherePrometric test centers; any center offering the NPTE, whatever your state

Two navigation rules catch people out. You can move freely within a section and mark questions to revisit, but once you end a section you can't go back to it. And because the clock runs across the whole exam, time you save in one section carries over to the next. At about 80 seconds per question on average, pacing matters. How many questions are on the NPTE breaks the format down further.

  • 225 questions in 5 sections
  • 5 hours of testing time, not timed per section
  • Scheduled break after section 2, not deducted from your time
  • 5 h 30 min total appointment

What's on the NPTE: the content outline

FSBPT's content outline, in effect since January 2024, splits the 180 scored questions across body systems and non-system topics. Musculoskeletal (44–54 items) and neuromuscular/nervous systems (39–48) together make up about half of the scored exam. Cardiovascular and pulmonary (22–27) come next, then six smaller systems, system interactions, and five non-system areas worth 21–29 items.

How the NPTE is scored

Your raw score (correct answers out of the 180 scored items) is converted to a scaled score from 200 to 800, and 600 is the pass mark for every jurisdiction. The conversion adjusts for small differences in difficulty between exam forms. FSBPT says the number of correct answers needed for 600 generally varies by only one to two questions between forms. There's no penalty for wrong answers, so answer everything. The NPTE passing score guide explains scaled scoring, why 600 isn't a percentage, and why two scores below 400 matter.

NPTE exam dates for 2026 and 2027

The NPTE-PT is offered on fixed dates four times a year, in January, April, July and October. The next 2027 date is January 26–27, 2027, with a registration and payment deadline of December 22, 2026. Registration closes about five weeks before each window, and your state must approve you about a week after that.

All twelve windows through 2028, with state-approval deadlines and score release dates, are on the NPTE exam dates page. For the registration steps, see how to register for the NPTE.

How much does the NPTE cost?

NPTE-PT fees (FSBPT and Prometric)
FeeAmount
FSBPT exam registration$485
FSBPT processing fee1.6%, rounded up
Prometric sitting fee (PT)$112
Score transfer to another state$90
FSBPT PEAT practice exam (optional)$99
State licensure applicationset by your state

Rescheduling with Prometric is free 30 or more days out, $60 at 5–29 days, and the full sitting fee inside 5 days. Withdrawing from FSBPT costs $50 (30+ days before the exam) or $120 (fewer than 30 days). Fees are from FSBPT's registration and payment page and the Candidate Handbook.

Who can take the NPTE?

To sit the NPTE-PT, FSBPT requires that you:

  • are at least 18 years old;
  • have (or are about to receive) a degree from a CAPTE-accredited PT program, or an education found substantially equivalent by a recognized credentialing agency;
  • have registered and paid, with no other open registration at the same level;
  • have no open security investigation or unfinished sanctions; and
  • have been approved by the licensing authority in the jurisdiction where you want to be licensed.

Graduates of CAPTE-accredited programs can sit up to 90 days before their graduation date once the school has validated it, though some states are stricter. If you didn't attend a CAPTE-accredited program, including if you were educated outside the US, your education must be found substantially equivalent and your state must approve you for the exam. FSBPT advises contacting your jurisdiction before you register. See can you take the NPTE before you graduate? for every state's early-testing rule and the NPTE path for internationally educated PTs.

What happens on NPTE test day?

The full checklist is in our NPTE test day guide. The essentials:

  1. Arrive at least 30 minutes early. Your belongings go in a locker.
  2. Bring two IDs that exactly match the name on your ATT letter: a primary government-issued photo ID with signature (such as a driver's license or passport) and a secondary ID with your name and signature (such as a credit card). If the names don't match, you won't be allowed to test, and you'll be marked a no-show.
  3. Check-in includes an ID scan, photo, signature, digital fingerprint and a metal-detecting wand.
  4. Leave your phone alone, even on breaks. Accessing it ends your exam immediately, and that attempt counts toward your lifetime limit.
  5. You may bring soft earplugs (they'll be inspected) and water in a clear, label-free container. An erasable note board is provided.

If you have a disability, request accommodations when you register; see NPTE testing accommodations.

When do you get NPTE results?

Your pass/fail status is posted to your FSBPT dashboard, and sent to your state, about five business days after your testing window ends. The free report with your numeric scaled score follows about ten business days after your exam date and stays free for 30 days. Exact dates for every window are in when do NPTE scores come out, and how to read your NPTE score report explains what each report shows. If you passed, contact your state board to finish licensure; what happens after you pass covers score transfers and the PT Compact. If you're moving later, the PT Compact may let you practice in another member state through a compact privilege.

What if you don't pass?

You can register again about 15 days after an unsuccessful attempt, within FSBPT's limits: no more than three consecutive attempts in 12 months, six attempts in a lifetime, and no further attempts after two scores below 400. Some states set stricter limits. Read the NPTE retake rules and what happens if you fail the NPTE before you re-register.

NPTE pass rates

In 2025, 85% of US-educated candidates passed on their first attempt, compared with 39% of candidates educated outside the US (FSBPT exam year report). Counting retakes, nearly all graduates of accredited US programs eventually pass. Look up any program on our NPTE pass rates by school table, or read is the NPTE hard? for what the numbers mean for you.

How to prepare for the NPTE

  1. Take a baseline test. Our free 50-question NPTE practice test is weighted to the blueprint and explains every answer, and 62 more practice questions by content area let you target weak systems.
  2. Pick your window and plan backwards. Use a 12-, 8- or 6-week NPTE study plan that gives each system time in proportion to its weight.
  3. Practice questions the right way. Review every explanation, including the ones you got right. Our practice-question method shows how.
  4. Sit full-length, timed exams in the last month to build five-hour stamina. FSBPT's PEAT includes a retired NPTE form, and our free trial includes a full-length 225-question exam.
